Notes:
This is the testimony of Hartog Polak, a survivor of this transport. In addition to his name, date of birth, and address, the document includes information about at least 18 other men whom he encountered as a Nazi prisoner. These include 1 man who was in Sakrau, 2 who died in Klein Mangersdorf, 6 who were in Neukirch, and 2 who were in Dörnhau. In addition, he provided information about four people who were on the transport out of Westerbork. The information about each person varies and can include last name, first name, date of birth or age, place of origin, and fate.
