This is the testiony of Lea Reisner-Beialowitz, a survivor of Sobibor. In addition to providing her place of birth and age at the time of her deportaton, the document also provides the names of two of her fellow prisoners: a cousin killed at Janowitz and a fellow Sobibor survivor whom she later married.
