This is a testimony of a survivor of the October 19, 1943, transport from Westerbork to Auschwitz. In addition to the information about him in the title, the document also includes his prewar and postwar addresses and his prisoner number at Auschwitz-Birkenau. He mentiones but does not name his wife and four children as well as two fellow prisoners, both men, who died at Jawischowitz. He also identifies by name nine fellow survivors who were at Birkenau with him after liberation. He provides both first and last names for most of them and also provides there places of residence after the war.
