If we piece the various fields in this database entry back together, we see that the abbreviation "SULZF" appears twice: once in an address field, and once spread across the first address field (SU) and the ghetto note field (LZF). This tells me that he likely lived at Adolf-Hitler-Strasse 1 (as you entered it into his profile) and was deported in March from Sulzfelderstrasse 36/38.

Okay for Jakub/Jakob's family I found his father Chaim Mendel b. 14/08/1901 or 1902, his mother Matylda b. 1899, and his sisters Sara born 18/2/1926 and Chana born 26/10/1929, and his broth Izack born 3/01/1934. They all share the same ghetto notes and deportation dates and according to the secondary sources the family would have been sent to Chelmno and killed in the gas vans.

Secondary sources indicate that there were daily transports leaving the Lodz ghetto to Chelmno during the month of March 1942, and there was an overwhelming likelihood that Jakub would have been killed upon arrival at Chelmno. Therefore, I've entered the place and date of death accordingly.
