School #6 was located at Ulica Limanoskiego 45. The street name was changed to Alexanderhofstrasse when German occupation began in 1939. See

I'm pretty sure the Sara I found is the same Sara that signed the albumn because she would have been the right age to attend school #6. Sara Lea Bursztyn was born on 20/03/1930 and she lived at Reiterstrasse 25 flat 11 and Reitersrasse 21. She was deported on September 12, 1942.

I found Fajga Bursztyn born in 1905 and had a ghetto note of WYM 03/05/1943 and Izrael born 09/03/1922 all living at the same address. I'm guessing they were Sara's mother and brother. Fajga also has the last name of Majerowicz so I'm thinking that was her maiden name.
Sara was deported on September 12, 1942 to Chelmno where she perished in the gas vans. Her brother was also deported with her.
