School #6 was located at Ulica Limanoskiego 45. The street name was changed to Alexanderhofstrasse when German occupation began in 1939. See

I did a search for Jakub Fusenbojm and didnt find anything so checked the "Sounds similar" box and found Jakub BOKSENBAUM
He was born in 1930 and he lived at Insel Strasse 24 11.

I agree that you've found the right person--the characters for B and F in Yiddish are relatively similar, and bojm/baum are the same word ("tree") in Yiddish and German/Polish, respectively, so the last name discrepancy is not a problem.
