I found the birthdate and name by searching the name on the museum database. The time is accurate to the deportation and what his age was so it is accurate. 

For the birthplace I Google searched "Chaim Kozienicki+Place of Birth" and it is the second choice down. It is from the USHMM websiteso I feel confident the information is correct.

All of this information was found on a USHMM website and verified. I think Chaim may have written a book, not very popular, but I'm still trying to verify that information.

Great work. I wouldn't count the movement into the ghetto as a deportation date, since in this stage we're looking at what happened after Chaim went into the ghetto, so please remove the 1940 dates from that section. Also, please add Stutthof as a camp to which Chaim was transferred, and please list in the comments some notes for other users who might not read the full pages of Chaim's story which dates go with which places.

There's one more change of address that you'll find if you search in the stage 1 database--February 8, 1944, to Blattbinderstrasse 3.
