Includes date, prisoner and block number, and where in the barracks the person died (e.g., "K.B." or "Arb.Kdo." - Krankenbau or labor commando). A Polish and a German-language cover letter precede the document; the German document, which is a 1960 description of its categories and abbreviations by Felix Rausch, the former prisoner-scribe in the hospital (Krankenbau). He indicates that the document/death book is the complete death book of Lager Auschwitz III - Monowitz.

The death registry becomes more specific in latter half, when it begins to include prisoners' names, date of birth and birthplace, cause of deth.
End-date of book not clear - could be 15th or 17th of Jan. 1944.. The scribe writes in 1960 that the register/book was filled out until the last day (of the camp), 17 January 1945; the camp was evacuated on January 18. He further qualifies the number of deaths that may be derived from this list, writing "Im Totenbuch sind nur die Toten erhalten, welche die Leichenkammer des Krankenbaus passiert haben. Haeftlinge, welche zur 'Sonderbehandlung' nach Birkenau gingen, sind nicht darin eingetragen". [Only the dead who ended up in the ?morgue of the Monowitz hospital were recorded in this book. Prisoners who were sent to Birkenau for "special treatment" were not entered [into this book]."
