I am adding his street address to the appropriate fields; though the numbers surrounding SIERPNIA were entered in a non-traditional manner; so, I entered the complete address with numbers in the OTHER STREET NAME field. In addition, I've added the transport number as it appears in the database. There is not enough information to deduce whether he was sent to Chelmno or elsewhere.

Looking at the full list of Lodz street names in German and Polish (http://kehilalinks.jewishgen.org/Lodz/streets-index1.htm) it appears that "Sierpnia 6-ego" was the name of the street being referenced--so 6, rather than being a street or apartment number, is part of the street name. Your decision to place the name in the "other street name" field was correct. I hope we are able to find more information about him soon.
