- Source Institution: Zydowski Instytut Historyczny (i.e. Jewish Historical Institute (JHI), ul. Tlomackie 3/5, 00-090 Warsaw, Poland), Record Group 201.
- The cards were rescued by unknown person and have obvious signs of fire damage and therefore were not microfilmed (the pages are very dark and partly burnt). JHI prepared a computer database containing information from the cards: last name, first and middle name, the year of birth, place of birth, father's name, mother's name, address, the date of death, the number of the death certificate, marital status, occupation, citizenship, cause of death, the coexisting illnesses and the name of the home doctor. Not all sections of the death certificates were filled out. The United States Holocaust Memorial Museum Archives received the collection from the Zydowski Instytut Historyczny via the United States Holocaust Museum International Archives Project in September 2007.

Abstract:
- The collection consists of a database recording information from 10,055 death certificates completed and signed by the Jewish doctors in the Warsaw Ghetto 1939-1941 for the office "Wydzial Statystyczny Zarzadu Miejskiego w miescie Warszawie" [= "Statistical Department of the Council of Warsaw"] for statistical and census purposes for the City of Warsaw. Facsimiles of the certificates themselves are not part of this collection held by the USHMM Archives due to the poor condition of the originals held by the Zydowski Instytut Historyczny (i.e. Jewish Historical Institute).
- The finding aid located in a 3-ring binder in USHMM Library-Archives Reading Room contains an alphabetical columnar printout from the database which includes only the following information from the database: last name [= Nazwisko], first and middle name [= Imiona], the year of birth [= Rok urodzenia], place of birth [= Miejsce urodzenia], father's name [= Imie ojca], mother's name [= Imie matki], date of death [Data zgonu] and file number [= Sygnatura]. Information on address, number of death certificate, marital status, occupation, citizenship, cause of death, the coexisting illnesses and the name of the home doctor are not included in this printout.
